I don't really worry about it, as I use most of my dolls for photography. Personally, I think using them for sex puts way more stress on a doll than posing them for photos. If anything, I worry about the joints becoming too loose over time. My first doll, Anastasia joint's in her knees aren't as tight, so standing her is a bit more of a challenge than when I first got her, but still doable.

I know the feeling. I am pretty freaked out about not damaging my doll as well (still?).
There are some basic hints about dressing up your doll I picked up on the forum.
One basic thing is you dress her without her head on.
You can protect her hand by holding her hand in your kinda if you pull something over her arm like sleeves. I additionally do first put socks on her hands to be sure a finger can not stick out. But again, for this you do aforementioned. I basically put my hand in the sock while I hold her hand.
Well, there are people with much more experience than me that might give you some additional hints.

I don't worry too much about damage as Anastastia, Anya and Ayumi have taken falls. Surprisingly, Ayumi fell on dirt and cement and not get one single scratch, maybe that's why they are called Irontech dolls. As you can see, she gives the thumbs up, I am all right, It's only a flesh wound. See Monty Python and the Holy Grail.
