They add a degree of realism, more range of motion, and you go from a goose neck to a hinged neck. The hinged neck is much better as it is quiet upon moving and it holds the pose much better. I have owned both now so I can tell you it is worth it! I don't have a side by side comparison of a doll without the shrugging shoulders vs with shrugging shoulders. However, here is a pic of my girl with her shoulders in the neutral position compared to raised up. If you get a skeleton without the shrug option, she will just be locked in the neutral position.
